Health Support Services
"I would like some advice regarding some illnesses."

Provide advice and support on specific illnesses.
            
Home Nursing Service
"My family member is sick and cannot come out from the house. He/She needs some nursing care."

Home Nursing Service provides nurses to assist the sick person who is home bound by providing nursing care and training home caregivers on how to care for their family members at home.              
            
Home Medical Service
"My family member is sick and cannot come out from the house. He/She needs some nursing care."

Home Medical Service provides ongoing medical care to the homebound older person through regular home visits. This service, usually comprising of a doctor, nurse and social worker.             
            
Community Hospital

Community hospital is an intermediate care facility between an acute hospital and community services.  It provides continuation of care from acute hospitals. The objective of community hospital is to promote functional independence of the older persons. Hence, it caters to those who require longer inpatient stay for rehabilitation, nursing and respite care after treatment in the acute hospitals before they are discharged back home.            
            
Hospice Home Care
"My parent(s) is/are terminally ill…where can I seek help & advice on how to care for them during this period of time?"

It is a 24-hour support service that reaches homebound patients who require medical and nursing care to control symptoms and manage pain. The visiting healthcare professionals including doctor, nurse and social worker also monitor the patients' condition and provide necessary training and support to the family.  The service is free but patients/families are required to purchase their own medication and dressings.

Hospice Day Care
"My parent(s) is/are terminally ill…where can I seek help & advice on how to care for them during this period of time?"

The Centre operates during weekdays. It provides support to persons in the early stages of terminal illness and/or those who are still able to cope with traveling. The programme includes rehabilitation and social/recreational activities to promote physical and social emotional well-being of the patients.

Hospice In-patient Care
"My parent(s) is/are terminally ill…where can I seek help & advice on how to care for them during this period of time?"

It provides physical and psychosocial care to patients, psychosocial support to patients' families and caregivers.  In addition, education and training in palliative care for healthcare professionals and the public are also offered. In an in-patient hospice, patients undergo treatment for symptom control (better control of pain, nausea or vomiting), respite care (short-term care to give care-givers a break) and terminal care (for patients who are in the very final stages of their illness). Patient must be certified by a medical doctor to be suffering from advanced and progressive disease, and the prognosis for the patient is less than 3 months before admission to the hospice.            
            
Medical Social Services
"I am currently unemployed can't afford my medical bills and I need help."

The Medical Social Workers (MSWs) attends to patients and their families who have difficulties coping with their medical bills, emotional, psychological, social and care problems arising from ill health and traumatic injuries.  As an inter-disciplinary team player, MSW collaborates with the healthcare team and community resources to provide patients and their families holistic care.  Please approach the Medical Social Services Department at the respective hospitals for assistance.
